The following forms are from the CBSA's or CFIA's web sites. You may wish to check with them in case they have updated the forms. B13A - Export Document (PDF) CCI - Canada Customs Invoice (pdf format) NAFTA Certificate (pdf format) Import Declaration This was the latest version available on CFIA's site but apparently is not the accepted version. If you use this form please white-out the part that says "fruit and vegetables" and change it to "food" or something to that nature. If you need help with some of the finer points on this form you may want to call your member of parliament and ask them because some of the questions CFIA hasn't been able to answer themselves.
