Supplier: This is the name and address of the person or company you purchased the goods from. Consignee: This is the company or person that is importing the goods into Canada.

Date of Shipment:
This is the date that the shipment left the supplier.

Reference Numbers:
Include PO Numbers and other numbers that may be useful to identify the shipment

Carrier:
The name of the carrier and the tracking number if available.

Currency:
The currency the values are in -- generally USD or CAD.

Description of goods, value, number of pieces and the country the goods were manufactured or grown in.

Check here if the goods were manufactured or grown in the USA. If the goods are of USA origin you will need a NAFTA Certificate if the value is greater than $1600.00 CDN. If the value is less than $1600 you will have to get the statement on the invoice signed.
